Write Off - Root causes & Reason codes within Supply IQ WO Workbench module are not being saved

See ticket enclosed. 

"The Root causes & Reason codes within Supply IQ WO Workbench module are not being saved, even though these information has been filled in WO Workbench module. As you can see in the attached screen shots, the AMP Planner responsible for this action filled both root cause & reason code for a specific item which has not been saved in SIQ WO Workbench module."
