Supply Alert - Have a blank or NA option for Secondary Root Cause

Problem Statement

In the SA page, the "Secondary Root Cause" does not have a blank or NA option. Since the secondary RC will not always be needed, it needs to have an option to let the user select that it is now known.

 Acceptance Criteria

 Secondary Root Cause has a "blank" or "NA" option available in the drop down
