Audit: Add to i buttons for Rapid Response Team (RRT) and Next Steps

1. Add i button next to 'RRT Proposed?' that reads "If no RRT add note to "DSRT Next Steps section" 

2. Add i button next to title of "DSRT Next Steps/Action Items" box that reads "If no RRT proposed add/attach concluding note to this section"
